Wirksamkeitsforschung

Vergleichende Wirksamkeitsforschung (comparative effectiveness research) schafft Evidenz zum Nutzen und Schaden verschiedener diagnostischer und therapeutischer Massnahmen mit dem Ziel, die medizinische Entscheidungsfindung zu verbessern. Die Evidenz stammt aus randomisierten klinischen Studien, und pragmatischen Studien (pragmatic trials), sowie aus Beobachtungsstudien. Diese Evidenz kann in systematischen Übersichtsarbeiten und Meta-Analysen zusammengefasst werden. ceb besitzt das Methodikwissen zur Durchführung unterschiedlichster klinischer Studientypen.

Evidenzsynthese

Wir erstellen systematische Übersichtsarbeiten und Meta-Analysen in zahlreichen Gebieten der Medizin zur Untersuchung von Medikamenten, medizinischen Apparaten, Verfahren, diagnostischen Tests und komplexen Interventionen.

Hierzu verwenden wir verschiedene spezifische Methoden, direkte und indirekte Vergleiche (die vom Institutsleiter entwickelte Bucher Methode), Netzwerk-Meta-Analysen, aggregierte und individuelle Patientendaten-Meta-Analysen, sowie Meta-Analysen von diagnostischen Tests.

Datenanalysen von Beobachungsstudien

Wir arbeiten mit Daten verschiedener nationalen und internationalen Beobachtungsstudien, sowie mit Routinedaten und erforschen prognostische Faktoren sowie die Wirksamkeit und Langzeitsicherheit von Medikamenten. ceb hat langjährige Erfahrung mit der Analyse von Kohortenstudien, insbesondere im Bereich der HIV-Infektion. Das Datenzentrum der Schweizerischen Transplantationskohorten-Studie ist mit dem Institut assoziiert.


Pragmatic trials

Pragmatische Studien (Pragmatic trials) versuchen eine bestmögliche Anwendbarkeit der Studienergebnisse für Alltagsbedingungen zu erreichen. Wir führen pragmatische randomisierte kontrollierte Studien, welche auf routinemässig erhobenen Registerdaten beruhen oder in hochstandardisierte Kohortenstudien eingebaut werden. Dies ermöglicht uns, den Nutzen von Interventionen besser unter klinischen Alltagsbedingungen zu untersuchen, sowie die Sammlung von Daten rascher und effizienter zu gestalten.


Methodologische Forschung, Research on Research

Die Entwicklung und Verbesserung der Methodik angewandter patienten-orientierter Forschung ist einer von mehreren Schwerpunkten des ceb. Hierbei erforschen wir die Struktur der medizinischen Forschung (Research on Research) und versuchen systembezogene Verbesserungen zu erreichen.

  • ceb untersucht Gründe für einen vorzeitigen Abbruch von klinischen Studien. Wir erarbeiten Strategien zur Abbruchsvermeidung, damit klinische Fragestellungen schlüssig beantwortet und Forschungsmittel optimal eingesetzt werden.
  • Wir untersuchen, wie die Qualität und Aussagekraft von routinemässig erhobenen Daten und Analysen von Beobachtungsstudien verbessert und verlässliche Information zur Qualität der Versorgung und Langzeitsicherheit von Interventionen gewonnen werden können.
  • ceb engagiert sich in Initiativen zur Verbesserung der Qualität von wissenschaftlichen Forschungsberichten um Forschungsmittel effizienter einzusetzen und wissenschaftliche Erkenntnisse optimal zu nutzen.
  • Wir setzen uns für die Schaffung von evidenz-basierten Standards und für eine transparente klinische Forschung ein, welche die Reproduzierbarkeit von klinischen Forschungsergebnissen sowie die Verschwendung von Forschungsressourcen durch fehlende Standardisierung vermeiden soll.

Area of Research

The main focus is on empirical mixed methods research of randomised controlled trials, especially on various types of trial discontinuation, patient recruitment, resource use and costs, subgroup analysis, loss to follow-up, patient reported outcomes and protocol reporting. We are developing tools for better recruitment prediction, budget planning and judging the credibility of subgroup effects. In addition, we are conducting various types of systematic reviews and meta-analyses (aggregate data, individual patient data, network) in the fields of respiratory diseases and intensive care.
